Solution and Benefits of the Program

Given the above facts regarding the "keepsake fetal video" programs, Dr. DeVore has created a new program that addresses all of the above issues.  The program is called the 3D/4D Fetal Heart and Face Screening Program.  Because of the time during pregnancy when this new ultrasound is performed (24 to 32 weeks of pregnancy), it is ideal for optimal 3D/4D imaging of the fetal heart and facial features.

This illustrates the time during pregnancy when the 3D/4D Fetal Heart and Face Screening ultrasound study would be performed.

The 3D/4D Fetal Heart and Face Screening Program is described as follows:

Examination of the Fetal Heart

This will focus on the fetal heart using 3D/4D ultrasound. This is an area that is most difficult for sonographers and physicians to examine during the second-trimester Screening ultrasound evaluation of the fetus. The technique that will be used will be the 3D/4D acquisition of the heart that can be displayed as a Tomographic Ultrasound Image, which was recently described by Dr. DeVore in the medical literature. This allows Dr. DeVore to view multiple sequential views of the fetal heart and identify abnormal anatomy.

This is a TUI image from a 4D STIC acquisition. It reveals all of the necessary views of the fetal heart to identify major malformations.

Examination of the Fetal Face

This will consist of 3D/4D surface renderings of the fetal face. If the fetus is cooperative, the images should have the appearance of a newborn baby.

The image on the left was obtained several weeks before birth. Notice that the two images are quite similar.

Are There Reasons Why the Ultrasound May Not be Successful as far as examining the Heart or Facial Features?

The success of the 3D/4D Fetal Heart and Face Screening Program is dependent upon several factors:

The position of the fetus. If the spine is up or the face is turned towards the back of the mother, the heart and face cannot be optimally imaged.

If the patient is overweight, this may interfere with the image quality.

If there is umbilical cord or hands and arms in front of the fetal face, this may prevent the examiner from obtaining a 3D/4D image of the fetal face.

Evaluation of Fetal Growth

In addition to screening for heart and facial abnormalities, measurements of the head, abdomen, and femur will be obtained to evaluate fetal growth. This is important because 10% of fetuses do not grow properly during the latter third of the pregnancy. If this condition is identified, further ultrasound studies are warranted. To examine fetal growth properly, it is important that the patient has undergone a previous ultrasound, either in the first or second trimester of pregnancy, to determine the gestational age of the fetus.

Who Will Perform and Interpret the Examination?

A specially trained fetal sonographer, with expertise in 3D/4D evaluation, will perform the study.

The images of the fetal heart and face will be reviewed by Dr. DeVore within 24 to 48 hours after the completion of the examination.

A report will be mailed to your physician regarding the results of the study.

If a malformation or other problem is identified, Dr. DeVore will contact your physician, as well as discuss the implications of the findings with you.

Images and Photos for the Family

You will be provided a DVD of the examination as well as select photos at no additional charge to you.  If you would like a videotape, bring a blank tape with you and a recording will be made during the examination.

Who Can Attend the Examination?

You will be able to bring family members with you to the examination. However, children under the age of 5 are asked to be accompanied by another adult.

Are There Any Requirements To Undergo This Type of Study?

The only requirement to qualify for this type of examination is that you must have undergone a previous ultrasound during your pregnancy that was provided by your obstetrician or another physician. If this has not been done, then you would need to speak with Dr. DeVore's office regarding a more comprehensive examination.

Summary of Benefits

Dr. DeVore believes that this new screening program has the following benefits:

1. Meets the needs of the patient and family who desire 3D/4D imaging of the facial features of their unborn baby.

2. Promotes the prenatal detection of fetal heart and facial abnormalities that are often missed during the routine screening (standard) examination of the fetus (16-22 weeks).

3. May identify unexpected growth abnormalities that would require further attention by your physician.

4. Is affordable for most patients.
